INTRODUCTION AND AIMS: Arterial hypertension is a global medical and social problem because of prevalence of high morbidity and mortality of population. There is a hypothesis of fetal programming: low birth weight (LBW) can cause high blood pressure (BP) due to low number of renal glomeruli at the birth. The aim of research was to determine the criterion of low birth weight as a predictor of chronic kidney disease. The objectives were: 1. Study the frequency of low birth weight among children suffering from kidney diseases 2. Evaluate glomerular filtration rate of children with low birth weight 3. Determinate the frequency of hypertension and low birth weight among young people METHODS: 1. 64 medical cards of children suffering from kidney diseases were analyzed for LBW and present kidney function. 2.788 young people (average age 20,1±0,2 y.o) were screened for BP and questioned. The questionnaire was developed by K/DOQI, added-on by us. Those, who had a high BP or LBW, were selected for further investigation according to the following including criteria: 1. BP > 140/90 mm.Hg (3-times measurement); 2. Birth weight < 2500 g. Then they were tested for albuminuria and serum creatinine (SeCr). Used interventions: measurement of BP; automatic analysis for albuminuria; test for SeCr; estimation of GFR by Cockrouft-Gault. RESULTS: Among children with kidney disease LBW was found in 35.9%. 78% among children with LBW have glomerular diseases (p<0,003). 21,7% of LBW children have high BP and 56,3% urine changes. 18,8% have hyperfiltration and 45% - low GFR. The prevalence of high BP among young people is 1.5% what correlates with world data in this age group. The prevalence of LBW is 6.2% and people with high BP and LBW simultaneously are 0.5%. Interestingly, results of laboratory tests and estimation GFR showed that in the group of high BP 50% have high GFR and 50% - low CFR. Moreover, it is notable that 100% of them have albuminuria. In the group of LBW-people 66, 6% have low GFR and so many have albuminuria. CONCLUSIONS: 1. Among children with kidney disease LBW was found in 35.9%. High BP among children with LBW was observed in 21.7%; decreased GFR in 45% and hyperfiltration in 19% cases.2. The prevalence of hypertension in young people is 1.5 % and correlates with world data in this age group; the number of persons born with low birth weight is - 6.2%. There is no clear link between LBW and hypertension in young population group and follow up monitoring needed.3. Albuminuria has been found 100% in high BP and 66% in LBW-groups. LBW can be considered as a predictor of arterial hypertension because of mild reduction of GFR in 60% of selected persons. Glomerular hyperfiltration is more characteristic to high BP.
