This editorial refers to ‘Effect of influenza vaccination in patients with decompensated heart failure: a systematic review and meta-analysis, by N. Ivars-Obermeier et al., https://doi.org/10.1093/ejhf/xuaf025. Heart failure (HF) remains a significant global health burden, particularly among older adults, characterized by a relentless cycle of hospitalizations and high mortality rates.1 While chronic stable HF is well-managed with a variety of evidence-based therapies, the period following an episode of acute heart failure (AHF)—often referred to as the ‘vulnerable phase’—presents a distinct clinical challenge. During the months following discharge, patients face a heightened risk of rehospitalization and death, with 1-year mortality rates following an AHF episode estimated at approximately 30%. Ivars-Obermeier and colleagues present the first systematic review and meta-analysis specifically investigating the impact of influenza vaccination in this high-risk AHF population.2 Their findings provide compelling evidence that a simple, low-cost intervention—the annual flu shot—may be one of our most effective tools in navigating this vulnerable period (Figure 1).
