Key messagesWhat is the problem?• A low number of clinical trials (CT) in the territory of the Republic of Kazakhstan. The number of CT per million of the population in the Republic of Kazakhstan is 1.8, while in most OECD countries this indicator exceeds 30 CT per 1 million of the population.• Remaining bureaucratic barriers at the level of an expert body -the National Center for the Expertise of Medicines and Medical Devices (NCEM), a high frequency of refusals to approve applications for CT that go beyond traditional schemes (studies of a combination of medicines, etc.).• For a number of "regulated CTs" (CTs of vaccines, stem cells, genetic technologies, drugs based on biotechnologies, nanotechnologies), the specifics of expert assessment of conducting CTs and the subsequent application of these technologies have not been clearly defined.• Insufficient transparency of licensing procedures for the CT sponsors and information support of CTs (due to the lack of a single portal for filing applications and a register of CTs).• A low level of motivation and, as a result, the reluctance of medical organizations (MOs) to participate in clinical trials.• Low motivation of doctors to participate in CT and the reluctance of patients to engage in CT. Journal of Health Development, Number 34 (Special Issue) 2019developing SOPs for conducting CT, a document establishing the procedure for working with confidential information, etc.• Digitalization of the process of filing applications for conducting CT and creating a register of CTs conducted on the territory of the Republic of Kazakhstan (with the ability to access information about CT in Open Access mode);• Effective information support of CT in a professional medical environment in Kazakhstan and abroad.Option 3. Implementation of effective mechanisms for the involvement of doctors in clinical trials, by:• Increasing the competence of doctors of the health organizations (first of all, the Republican, oblast and citylevel hospitals) on the issues of the CT methodology and good clinical practice (GCP); • Stabilization of the relationship «doctor-patient» on the basis of increasing communication skills and competencies in the field of medical ethics; introduction of preferences for patients in CT; development of SOPs according to the competence and subordination of a doctor within the framework of CT; • Taking organizational measures to involve doctors in CT based on effective time management in health organizations; redistribution of the functional of the doctor-researcher for the period of the CT; elimination of "routine" procedures due to the reorganization and restructuring of the health organizations through digitalization, the use of new technologies to support the doctor's functional in the health organizations. Vision on the implementation of policy optionsEach of these policy options can contribute to the development of the CT market in the Republic of Kazakhstan. However, given the differences in options in the direction of actions, the resources and methods used, these policy options can give a more substantial achievement of the goal of expanding the scope of clinical trials in the Republic of Kazakhstan with their combined implementation.
