Aim: We aimed to investigate the phenotypic and polysomnographic differences between positional and non-positional OSAS patients. Materials and Methods: We were examined the data of our sleep laboratory between 2005-2012 years. Positional sleep apnea was defined as a total apnea-hypopnea index (AHI) > 5 with a > 50% reduction in the AHI between the supine and non supine postures in OSAS patients. The polysomnographic data and demographic properties (waist and neck circumference and body mass index) were recorded. Results: The 604 patients with mean age of 48.25 ± 0.46 years were enrolled into this study. The patients were divided into three groups as positional sleep apnea, non-positional sleep apnea and control group. There were 510 OSAS patients and 50.39% of them detected as positional OSAS. The patients with positional sleep apnea were found as younger, taller, weaker and had a thinner waist and neck circumference. There were significant differences about REM, Stage 1 and Stage 3 features between positional and non-positional OSAS patients. Positional OSAS was found to be more in patients with mild to moderate OSAS. Conclusion: As a result of this study, the patients with positional sleep apnea were younger, taller, weaker and had a thinner waist and neck circumference. The obstructive apnea symptoms are with mild- moderate severity during the diagnosis of positional OSAS and the severity of the disease increased and changed with position. This increases the incidence of comorbidity. For this reason, positional OSAS should be evaluated as a different clinical entity and treatment plan should be made due to this.
