Objective: To assess ocular surface symptoms and tear-film parameters among young women with polycystic ovary syndrome (PCOS). Methods: A cross-sectional study was conducted among 112 women aged 18–35 years with confirmed PCOS at Mayo Hospital, Lahore. The sample size was calculated using OpenEpi version 3.01 based on a 92.1% anticipated prevalence of meibomian gland loss, 5% precision, and 95% confidence level. Women with confirmed PCOS were included, while those with ocular surgery, active ocular disease, contact lens use, autoimmune disease, pregnancy, or conditions/medications affecting tear-film function were excluded. OSDI(Ocular Surface Disease Index), TBUT (Tear Break-up Time), Schirmer I test, and corneal staining were assessed. Data were analyzed using SPSS version 26, with normality assessed by the Shapiro–Wilk test and appropriate parametric or non-parametric tests applied. Results: The mean age was 25.89 ± 3.93 years and mean OSDI score was 35.45 ± 14.98. Severe symptoms were present in 53.6% of participants. OSDI scores showed significant negative correlations with TBUT and Schirmer I values in both eyes (p < 0.001), while age, BMI, and PCOS duration showed no significant association. Conclusion: Women with PCOS showed a high frequency of ocular surface symptoms, with higher symptom severity associated with reduced tear-film stability and aqueous tear production. Ocular surface assessment may be considered in women with PCOS reporting dry-eye symptoms.
