Background: Chronic rhinosinusitis is one of the most prevalent chronic inflammatory diseases of the upper respiratory tract, affecting approximately 10-15% of the adult population worldwide. The disease significantly impairs quality of life, reduces work productivity, and imposes a substantial socioeconomic burden. Recent advances in endoscopic sinus surgery, biologic therapy, artificial intelligence, molecular diagnostics, and digital health technologies have transformed the diagnosis and management of CRS, enabling more personalized and evidence-based treatment strategies. Objective: This review aims to summarize contemporary approaches to the diagnosis and treatment of chronic rhinosinusitis, emphasizing the role of digital technologies, artificial intelligence, biologic therapies, precision medicine, and future innovations in rhinology. Methods: A comprehensive narrative review was performed using publications retrieved from PubMed, Scopus, Web of Science, Embase, and the Cochrane Library. Randomized controlled trials, systematic reviews, meta-analyses, international consensus statements, and clinical practice guidelines published mainly between 2020 and 2026 were analyzed to evaluate current evidence regarding CRS management. Results: Contemporary management of CRS has evolved from conventional symptom-based treatment toward personalized medicine based on inflammatory endotypes, biomarkers, and molecular mechanisms. Functional endoscopic sinus surgery remains the gold standard for medically refractory CRS, while biologic therapies targeting type 2 inflammation have significantly improved outcomes in patients with severe eosinophilic disease and nasal polyposis. Artificial intelligence has demonstrated promising applications in radiologic image analysis, endoscopic image recognition, surgical navigation, predictive modeling, and individualized therapeutic decision-making. Telemedicine, wearable technologies, and digital symptommonitoring platforms further support long-term disease management and improve patient engagement. Conclusion: Integration of precision medicine, biologic therapy, artificial intelligence, and digital technologies is reshaping modern rhinology. Future multidisciplinary approaches combining molecular diagnostics, advanced surgical techniques, and intelligent clinical decision-support systems are expected to optimize individualized treatment and improve long-term outcomes for patients with chronic rhinosinusitis.
