Relevance: Cervical self-sampling is a great opportunity to enhance the screening uptake for the detection of cervical cancer, especially among the under-screened population. This approach helps maintain ease of use by enhancing patient privacy and making screenings more affordable. While human papillomavirus (HPV) genotyping on self-samples is widely accepted, the diagnostic accuracy of self-samples for detecting precancerous lesions (CIN, LSIL, HSIL) remains unclear. The study aimed to assess the diagnostic accuracy of cervical self-sampling methods for detecting precancerous cervical lesions compared with clinician-taken sampling. Materials and Methods: This systematic review analyzed studies published in English and indexed in PubMed, EBSCO, and Google Scholar databases till April 2025. Only original research articles comparing self-collected and clinician-collected samples were included in the review. The risk of bias in the selected studies was assessed using the Newcastle-Ottawa Scale (NOS). Results: Of the 1074 records initially identified, only 6 studies met the inclusion criteria. Among those six, five studies used liquid-based cytology (LBC), while one used biopsy as the reference test. The results varied across studies; one study (16.6%) reported an accuracy of 87% and concluded that it had high accuracy, sensitivity, and good specificity. Three studies (50%) demonstrated moderate diagnostic accuracy, while two studies (33.3%) reported low diagnostic accuracy. Conclusion: Overall, cervical self-sampling demonstrates low to moderate diagnostic accuracy in detecting cervical precancerous lesions. While it cannot be considered an independent, fully reliable screening method, its integration as an additional approach, combined with other screening tests, can help reach non-attenders or populations with limited access to healthcare services.
