Relevance: Liver metastases are a common complication of malignant tumors, significantly worsening patient prognosis. Radiological assessment of treatment response is crucial for disease monitoring and therapy optimization. The study aimed to analyze modern radiological methods for evaluating the response of liver metastases to systemic therapy, identifying their advantages and limitations.Materials and Methods: A systematic literature review was conducted following PRISMA 2020 guidelines. Publications from PubMed, Scopus, Web of Science, and eLibrary (2015-2025) were included, focusing on original studies, meta-analyses, and clinical guidelines. Results: A total of 42 publications were reviewed. The primary imaging modalities were computed tomography (CT) (80% of studies), magnetic resonance imaging (MRI) (30%), and positron emission tomography (PET-CT) (25%). RECIST 1.1 remains the standard assessment criterion but has limitations for targeted and immunotherapies. Morphological changes (Chun criteria), metabolic parameters (PET response criteria in solid tumor, PERCIST), and diffusion-weighted MRI (DWI) demonstrated high prognostic value. Promising areas include radiomics, machine learning algorithms, and hybrid imaging (PET-MRI).Conclusion: A comprehensive approach combining morphological, functional, and metabolic criteria improves the accuracy of treatment response assessment. The integration of artificial intelligence and radiomics holds potential to enhance diagnostic objectivity. Future research should focus on standardizing and integrating quantitative image analysis methods.
